This screen is accessed by two user types:
Tenants can access this form from the Welcome screen and from the Work Requests tab.
SKYLINE Users can access this screen when adding a new request from the FM/WO, Work Requests or FM/WO Summary screens.
NOTE You must own SKYLINE Web Work Requests to access this form.
Provide a Description of the Work, your Email Address, and any other necessary information. Click Submit.
Tenant Details - the top form section contains read-only Tenant information based on your login, such as, Occupant Name, Property, Unit Reference Number, and address.
Email Address - Enter your Email address for correspondence purposes. This Email is used to receive Work Request Confirmations if the Email Notification field (on this form) is set to Yes. You can also save this address as the default.
Contact Phone - Enter the best phone number for management or the service provider to reach you.
Email Notification - Set to Yes to automatically receive an e-mail containing the request details. The e-mail address on this screen is used for the automatic confirmation.
Permission to Enter - Select an option from the drop-down list to indicate whether a service provider has permission to access your unit. The code tells service personnel whether they have permission to enter the unit or if they must contact you first. Some examples include Call First, Use Master Key, Use Back Door, and See Building Manager. You can also use the Special Instructions field on this screen if the Permission to Enter list does not contain the information you require.
Description - Describe the problem you would like resolved. This is a required field.
Special Instructions - Enter any additional comments or instructions.
Submit - Click Submit when the form is complete. An email is sent confirming that the work request was submitted if the Email Notification field is Yes. A summary screen displays that you can print.
